While not technically in the Mid-Atlantic, Kentucky's Red River Gorge in Daniel Boone N.F. is a fairly reasonable drive for most in the region.
The Red River Gorge has the highest concentration of natural arches in the country outside of UT/AZ and has an abundance of natural bridges, caves, cliffs, waterfalls, and scenic overlooks. Embarrassingly, I never even knew this place existed until fairly recently.
I camped for three nights in the Zilpo Recreation Area at Cave Run Lake and also visited Broke Leg Falls, Sky Bridge, Whistling Arch, Princess Arch, Indian Arch, Cloudsplitter, Chimney Top, Lockegee Rock, Tater Knob, Natural Bridge State Park, and some lesser-known spots. Here are some of the highlights from my visit:

Looks spectacular! Nice photos too! Thanks. Red River Gorge is one of the finest natural treasures in the Eastern U.S. in my opinion. I wouldn't be surprised if it gets "upgraded" to national park status at some point in my lifetime.

We backpacked in the Red River Gorge and Clifty Wilderness several years ago - very interesting area. Clifty was very challenging in the trail finding aspect.

The entire length is over 300 miles and it runs through RRG. I wonder if it's popular among hikers and backpackers, or more under the radar like some of the longer trails in Pennsylvania.

Very, very late reply here. We did use some of the Sheltowee Trace.

If you liked RRG, go to Big South Fork Nat'l Recreation Area, probably the best kept secret in the national park system. Incredible place.
